Requirements
- DO, MD or MD-DO/PhD
- Board certified or board eligible in ophthalmology
- North Carolina unrestricted medical licensure or eligible
- Current Drug Enforcement Agency (DEA) and Department of Public Safety (DPS) registration in North Carolina
